Insurance and Payment Policy
I do not participate in any commercial insurance plans. However, if you have out-of-network benefits, you may still be eligible for insurance reimbursement for treatment.
Payment and Receipt: I collect payment from you at the time of service and provide a detailed receipt (a "superbill") that includes your diagnoses, treatments, and associated costs.
Submitting to Insurance: You submit this receipt to your insurance company along with their out-of-network reimbursement form (which you can obtain from them).
Reimbursement: Your insurance company will then process your claim and send you a reimbursement check, depending on your plan's coverage.
Understanding Your Coverage
Allowed Fee: The fee your insurance company considers reasonable for specific medical billing (CPT) codes.
Deductible: The amount you must pay before your insurance starts covering services.
Coinsurance: After meeting your deductible, this is the percentage of the allowed fee you are responsible for.
Out-of-Pocket Maximum: Once met, your insurance covers 100% of the allowed fee.
