This policy explains what information Axis Psychological Services (Dr. Jesse Finkelstein, PsyD, New York City) collects through axiscbt.com and how it is used. It covers this website only. Clinical records and anything you share in treatment are protected under HIPAA and described in the Notice of Privacy Practices you receive when treatment begins.

Consultation requests and scheduling

Booking and consultation requests are handled by SimplePractice, a HIPAA compliant practice management platform. Information you enter there, such as your name, contact details and reason for reaching out, is stored by SimplePractice and used only to respond to you and arrange care. SimplePractice's own privacy policy also applies. Please do not send detailed clinical information through general email or website forms.

Information collected automatically

Like most websites, axiscbt.com uses cookies and similar tools to understand how the site is used:

Google Analytics records pages visited, approximate location, device type and how visitors arrived.
Microsoft Clarity records how pages are used, such as clicks and scrolling, to help improve the layout.
Google Ads and Microsoft Advertising measure whether an ad led to a visit or a consultation request.

This information is used in aggregate to improve the website and measure advertising. It is not used to identify you, and it is never sold.

Reporting tools

The practice uses an internal reporting tool that reads aggregated statistics from Google Analytics, Google Search Console and Google Ads to produce website performance reports. It has read only access, does not collect information from visitors, and does not access clinical or personal health information.

Your choices

You can block or delete cookies in your browser settings, use the Google Analytics opt-out browser add-on, and manage ad personalization in your Google and Microsoft account settings. The site works without these cookies.

Children

This website is not directed to children under 13, and we do not knowingly collect information from them online.

Insurance and Payment Policy

I do not participate in any commercial insurance plans. However, if you have out-of-network benefits, you may still be eligible for insurance reimbursement for treatment.

‍Payment and Receipt: I collect payment from you at the time of service and provide a detailed receipt (a "superbill") that includes your diagnoses, treatments, and associated costs.

‍Submitting to Insurance: You submit this receipt to your insurance company along with their out-of-network reimbursement form (which you can obtain from them).

‍Reimbursement: If the claim is eligible, your insurer will reimburse according to your plan's terms.

‍Understanding Your Coverage‍

‍Allowed Fee: The fee your insurance company considers reasonable for specific medical billing (CPT) codes.

‍Deductible: The amount you must pay before your insurance starts covering services.

‍Coinsurance: After meeting your deductible, this is the percentage of the allowed fee you are responsible for.

‍Out-of-Pocket Maximum: Your plan may set a limit on certain covered costs. Out-of-network services may have a separate limit or may not count toward the in-network limit. Amounts above the insurer's allowed amount and noncovered services may remain your responsibility. Check your plan's specific out-of-network benefits.
